We are looking forward to have you join us for the 3rd Annual Fountain City Tattoo Expo! Please complete all of the steps below in preparation for the event.
Step 1: Purchase Your Booth & Permit Fee
The artist booth only needs to be purchased by the person in charge of your booth. The permit fee and all other items on this page need to be purchased / completed by every artist attending.

Step 3: Complete Your Verification of Residency Form and Tattoo Artist Permit
*All forms must be completed and submitted by August 1st* *Please fill out forms completely and correctly* *Your Verification of Residency Form must be Notarized*
Once you have completed your Health Department Requirements and Verification of Residency Form, please upload all documents via the form below or email all documents to info@fountaincitytattooexpo.com. You may also mail to:
